Pre-Participation Physical Exam/ Medical Forms

In compliance with the NJCAA, the Finger Lakes Community College Sports Medicine Team requires the YEARLY completion of a Physical Examination prior to participating on an athletic team. FLCC uses SportsWare OnLine, a secure, encrypted, and HIPAA-compliant online data entry system to collect all required medical paperwork. Once all pre-participation paperwork is submitted and approved by the Athletic Trainer, the student-athlete will be fully cleared to participate.

Physicals:

All student-athletes are required by the NJCAA to have an approved SPORTS physical exam before they can participate on a Finger Lakes Community College athletic team.

There are two ways this can be accomplished:

  1. Our team doctors will be on campus at the start of fall training seasons to conduct physicals. If you have any pre-existing conditions that may cause concern (ex- prior surgery, cardiac tests, respiratory tests), please bring the appropriate records from your doctor regarding these issues indicating your clearance to participate.
  2. This physical costs $15 (cash or check only, checks can be made out to Rochester Regional Health).
  3. Another option is to take the Physical Examination Form to your own primary care physician or urgent care facility.

Training Room Hours:

Training room hours are dependent on the number of sports in season, competition and practice schedules, and the availability of the athletic training staff. A Certified Athletic Trainer will be present at all home Finger Lakes Community College athletic events.

Other Resources for Student-Athletes:

Student-athletes are also encouraged to review the college's policy regarding insurance for injuries.
